What does a CNL do?
- Coordinates care across a multi-disciplinary care environment for a distinct group of patients
- Communicates and collaborates with the entire multidisciplinary team
- Puts evidence-based practice into action at the bedside
- Collects and evaluates patient outcomes
- Assesses cohort risk
- Manages data collection and data analysis for specific cohort
- Provides clinical leadership to staff nurses through consultation, education, and mentoring
- Has the decision-making authority to change the plan of care when necessary
- May actively provide direct patient care in complex situations
