Academic Appeal Form
Students may appeal a course failure with this form.
Clinical Placement Information Form
This form is used to generate the Clinical Preceptor Agreement Letter. Graduate students may not participate in a clinical experience without a signed Clinical Preceptor Agreement returned to UMDNJ and following notification from the Office of Clinical Affairs.
Drug Testing Release Form
Some University-affiliated clinical facilities require students to undergo drug testing prior to placement at the site. For sites where drug testing is not offered please utilize this form.
Student Authorization for Criminal Background Check
In order to complete your educational program at UMDNJ, the performance of a completed criminal background check is required.

Application for Admission
Paper application for admission to School of Nursing Graduate & Doctoral academic programs. (BSN programs apply via Nursing CAS)
Admissions Deferment Form
Did you get accepted but can't start school yet? Use this form to postpone your start date for up to one year. We will no longer be offering deferred admission to any SN Program beyond Summer 2013.
Tuition Deposit Form
You'll need to pay your tutiton deposit before you can register for courses.
Request for Transfer Credit
Students requesting credits earned outside of SN must submit official transcripts, course descriptions and any additional documentation (i.e. syllabus, research papers, etc.)
Re-Evaluation of Admissions
Use this form to request an application be reviewed again after being previously advised to complete non-matriculated coursework and grades for your non-matriculated coursework have been officially posted to your academic transcript.
Students are normally required to register online at my.umdnj. If you are unable to register on-line due to a prerequisite or test score requirement, you will need to obtain written permission from your advisor using this form. If you are having technical difficulties registering online, please feel free to contact Enrollment Services at (973) 972-5454.
Change of Registration Form
Need to add or drop a course? You may do so online or by submitting this form. After the Add/Drop Period, students receive a 'W' grade. Students who are not registering for any courses, dropping all courses, or withdrawing from the School must use the Student Status form.
UMDNJ Cross Enrollment Form
This form must be used to cross-enroll in a course offered by a UMDNJ School other than SN. Your Academic Advisor will review course materials and determine if the course is appropriate for you.
Change of Student Program Status
All students must either be enrolled for the current term or upcoming term(s) or be placed on one of the following statuses:

Ready to graduate?
Application for Degree/Certificate
Almost completed all of your degree requirements? Let us know when you expect to graduate and whether you will attend Convocation or Commencement. Deadline to apply: For January 2013 Graduates: November 19, 2012;
For May 2013 and September 2013 Graduates: January 14, 2013
Certificate/Diploma Release Form
Once available, students may pick up their diplomas/certificates at the Enrollment Services Center or submit this form to have the diploma mailed to you directly. Students must be cleared of all holds in order to eligible to receive their diploma.
Change of Commencement Attendance Form
Students that indicated on their previously submitted “Application for Graduation” form that they were intending to attend the Commencement ceremony but no longer plan to attend are required to advise Enrollment Services.
Forms specific to Commencement or Convocation ceremonies may be found on the graduation site (academic attire, special needs, pin orders, etc)
Enrollment/Graduation Verification Request
Students in need of documentation for loan deferments, confirming their registration or graduation information, should complete this form.
Residency Policy and Application
This defines NJ residency to qualify students for in-state tuition rates and apply for a change to in-state residence for students previously classified as out-of-state.
Application for Replacement Diploma
To obtain a new diploma because of damage, loss, or name change.
Request official or unofficial copies of your transcripts.
Student printing deposit form
Students who have exceeded their free printing allotment, will need to recharge their printing account using this form.
