Dental Plan
UMDNJ will once again offer our students, postdoctoral fellows and their dependents the DeltaCare dental insurance plan for the 2009-10 academic year. Enrollment in this plan is voluntary; it is not required insurance.
Students and postdoctoral fellows interested in participating in the plan will enroll on-line, beginning June 8, through University Health Plans (UHP). The on-line enrollment form, plan benefits and highlights and a list of network dentists can be found by linking to http://universityhealthplans.com , and following the links to the UMDNJ dental insurance information.
The premiums are:
Student Only - $263.72
Student + 1 dependent - $502.84
Student + 2 or more dependents - $790.68
July 15, 2009 is the enrollment deadline for coverage from August 1, 2009 to July 31, 2010. Deadlines of August 15, 2009 and September 15, 2009 have been established as well for September 1 and October 1 effective dates respectively. Students who enroll after the first deadline of July 15 may not appear on their dentist's roster until the first week of the effective date, and their dentist will need to call DeltaCare to verify eligibility. The premium will not be pro-rated for the September and October effective dates. Health Insurance
All full-time UMDNJ students, as well as those part-time UMDNJ students who participate in clinical experiences as part of their educational programs, are required to be covered by health and accident insurance. The UMDNJ-School of Nursing separates student's enrollment eligibilities into two categories:
If you are a BSN student or a nurse anesthesia student, and you have not waived participation in the University Health Plan, you will be automatically enrolled in and charged for the University Health Plan as part of your tuition. The online Waiver can be found at www.universityhealthplans.com
If you are a School of Nursing Graduate student and need the University Health Plan, you must self enroll each year. The annual premium will not be charged to your tuition and you are responsible for payment. If you wish to voluntarily enroll in the University Health Plan, you must
complete an Enrollment form to purchase coverage. The Enrollment form is available at the School of Nursing or you can download it at: www.universityhealthplans.com
January 15th is the deadline to waive health insurance or you will be responsible for the coverage charge
